Dear Student, If you need to have your classes, materials, tests or examinations adapted to your needs, please read the following general information about how you can apply for adaptation measures. See below for detailed rules and all the necessary forms and documents.

The steps in the application process are as follows.

1. PREPARE YOUR APPLICATION FOR ADAPTATION MEASURES 

You need to prepare all the necessary documents and submit them to the Office for Persons with Disabilities. Alternatively, if you are attending classes online, you may send scanned copies by email to adaptacje@vizja.pl.

Documents required:

  • Application for Adaptation Measures (Form 2),
  • a copy of AEH's Data Privacy Notice (with your signature) and a statement of your consent to the processing of your personal data,
  • a notice of your (valid) disability determination decision and/or your medical record (this must be valid for 90 days on the date of your application).

Please note we will accept any documents in Polish only. All documents in a language other than Polish must be accompanied by certified translations issued by a sworn translator. Please do not send us photographs of any documents.

2. MEET OUR COMMITTEE

You will attend a meeting with our committee. The members of the committee will consider your application and will or will not recommend the adaptation measures you applied for. You can attend the meeting in person or online (via Teams).

3. WAIT FOR THE DEAN'S DECISION

Your application and the committee's opinion will be delivered to the Dean. The Dean will accept or reject your application.

4. YOUR ADAPTATION MEASURES FORM WILL BE ISSUED.

If the Dean has accepted your application, the Office for Persons with Disabilities will issue an Adaptation Measures Form [Polish: karta adaptacji]. This form will specify your adaptation measures. This form will not contain any information about your health status. It will only show your adaptation measures.

5. CONTACT YOUR LECTURER / TEACHER.

If you have received an Adaptation Measures Form, you should contact your lecturer (or teacher) directly and show them your Adaptation Measures Form. You must do so at least one week before the test or examination. The Office for Persons with Disabilities will not be involved in any arrangements between you and your lecturer (or teacher).


The committee meeting will take place on a date arranged with you by the committee.

Location:

On site: On the premises of the Office for Persons with Disabilities: Room 233 (2nd floor) The office will be open for document submission and meetings between 9 AM and 2 PM, from Monday to Friday (except Wednesdays).

Online: Via Teams: Between 9 AM and 2 PM, from Monday to Friday (except Wednesdays).
A staff member of the Office for Persons with Disabilities will arrange a Teams meeting with you.


In the 2023/2024 summer semester, applications may be submitted from  18 March 2024 to 26 April 2024.
